Dear Patient,

We are writing to remind you that Anchor Medical Associates will permanently close on June 30, 2025. We are deeply grateful for the trust you have placed in us over the years, and we want to ensure that your transition to new care is as smooth as possible. Please review the following important information and next steps:

Key Dates and Actions Before June 30, 2025 

  • Patient Portal Access Ends at 3:00 PM on June 30
    Please download any medical records or information from the patient portal before this time. Access will no longer be available after 3:00 PM.

  • Outstanding Orders
    Any lab, imaging, or consult orders not yet completed will need to be reordered by your new healthcare provider.

  • Unaddressed Results
    If you have pending results that have not been reviewed by an Anchor provider, please share them with your new clinician for follow-up.

  • Record Pickup Deadline
    If you requested medical records and were notified, they are ready to pickup at the office, they must be picked up by 3:00 PM on June 30.

  • Emergencies After Closure
    After June 30, if you are experiencing a medical emergency, do not wait—please call 911 or go directly to the nearest emergency room.

What Happens After Anchor Medical Closes

Arches Medical will serve as the custodian of medical records for former Anchor patients beginning July 1, 2025. Arches Medical was selected for their established relationships with many Anchor clinicians, their use of the same Athena electronic health system, and their commitment to continuity of care.

Your Options Moving Forward 

  • Requesting Medical Records
    After July 1, requests for medical records should be made through Arches Medical’s website:

    Note: Time-sensitive requests before July 1 should still be directed to Anchor Medical, but processing times may be delayed.

  • Continuing Care with Arches Medical
    If you are an adult patient (17+) and would like to continue your care with an Arches PCP at one of their seven primary care locations, please fill out this form:

    Or call 475-766-1041 to schedule an appointment.
    Patients transferring to Arches do not need to request their records separately.

 

  • Same-Day Care After July 1
    Arches Medical offers walk-in and telehealth services for all ages, including urgent care, sick visits, and short-term prescription refills at the following locations:

    • 982 Tiogue Ave, Coventry, RI 02816
