Following the announcement of Anchor Medical Associates’ June 30, 2025, closure, primary care practices have been reaching out to offer their support and willingness to accept new patients. Please be aware that provider availability and insurance participation may change. We recommend first contacting your insurance member service’s department for a listing of providers within your plan’s network that are accepting new patients.
It is important that patients obtain a copy of their medical record now, so they have them in hand to send or when visiting their new provider. The most efficient way that does not require a medical release form, and is a free service up until June 30, 2025, is to download and/or transmit medical records from their Anchor Patient Portal account. Be sure to first review the How to Guide located towards the bottom of this page or by clicking the Patient Forms link above.
If you do not have an active patient portal account, you can request a copy of your medical records by completing a medical records release form located towards the bottom of this page or by clicking the Patient Forms link above. There may a fee charged for this service.

Vaccines
NEW SHINGLES VACCINE
Have you heard about the new Shingles vaccine? It is called Shingrix, and it is 90% effective at preventing the painful shingles rash. It is recommended for everyone over 50, and even people with suppressed immune systems can get it.
It is a series of two shots. It is available in most pharmacies. Many insurances don’t cover all of the cost, so you should ask how much the injection will cost you. You may get a sore arm or a low grade fever that lasts 2-3 days after the injection.
